| Appointments If, for some reason, you cannot keep a scheduled appointment we ask that you notify us no later then 48 hours in advance. If you will be delayed for an appointment, please call as soon as possible. Do to our busy schedule we may need to reschedule your child's appointment. 1st. Appointment We encourage a parent of small children to accompany them during their initial visit. Do to the expansion of our office with a third chair and limited space in our operatory we do ask that all other siblings/guests remain in our waiting room. During subsequent appointments we will ask that parents and siblings remain in the waiting room to allow optimum care of all of our patients. |

| Payment Policies and Information We request that patients pay for their responsible out-of-pocket costs at the time services are rendered. We accept cash, personal checks and many major credit cards. Additionally, our office staff can discuss payment plans with you, when appropriate circumstances occur. |
